1. If you have visited these 15 places (addresses attached) between May 28 and June 6, please immediately report to your village (neighborhood), take the "four ones" health management measures (issuing a health notice, carrying out a health inquiry, checking the health code and taking a PCR test) as required, and cooperate with the epidemiological investigation: Zheng Jiaolan fruit store, Lianghui grocery store, Haoyikang Pharmacy, Guang Tou Lao restaurant, Dejing convenience store, Hexing farm restaurant, Hao Yun Lai chess and card house and the business building it locates, Xinnan Street of Beilian Village, Saizeriya Restaurant, Zhu Mei Xian wonton noodle shop, Dashi Mingjian Palace, Happy Hour, Shangdong Baozi International, Liaoji Beef Hot Pot and Fu Le Yi chess and card house.

2. Anyone conceals or makes a false report of their travels, or fails to implement the epidemic control measures as required by the city or the district and causes risks of transmission of COVID-19 or other serious consequences will be subject to legal liabilities according to the Law of the People's Republic of China on the Prevention and Control of Infectious Diseases, the Law of the People's Republic of China on Public Security Management and Punishment, the Law of the People's Republic of China on Emergency Response and the Regulations on Preparedness for and Response to Emergent Public Health Hazards.

Please cooperate with the investigation, truthfully provide your movement tracks and personal health information, and cooperate with the PCR testing, health monitoring and other measures to help win the fight.
